The undersigned (“Customer”) requests and authorizes R.I. Lampus Company and its drivers, vehicles, and equipment (“Lampus”) to leave the public roadway and travel onto private property at the delivery address above in order to deliver and/or place materials.
Customer understands that delivery trucks, boom trucks, and other heavy equipment are large and heavy, and that travel onto private property carries an inherent risk of damage. By signing below, Customer accepts full responsibility for, and releases, waives, and holds harmless Lampus from, any and all claims, damages, or costs arising from the delivery vehicle or equipment leaving the roadway and entering the property, including but not limited to damage to:
• concrete, asphalt/blacktop, paver, gravel, or other driveways and surfaces;
• underground or buried lines and systems — including water, gas, electric, sewer/septic, drainage, irrigation/sprinkler, invisible fence, and similar;
• lawns, landscaping, trees, curbs, sidewalks, retaining walls, and structures; and
• any other real or personal property on or under the site.
Customer confirms they are authorized to grant access to the property and that, to the best of their knowledge, they have identified any known underground utilities or hazards to the driver. Customer agrees that Lampus is not responsible for locating buried lines and that any decision to drive onto the property is made at Customer’s direction and risk.
This release is binding on the Customer and their heirs, successors, and assigns, and applies to this delivery and any repeat deliveries to this address unless revoked in writing.
